What is zolmitriptan?
Zolmitriptan is a second-generation triptan for the treatment of acute migraine attacks. It is available in various dosage forms: as a normal tablet, melt tablet and nasal spray.
The nasal spray is particularly advantageous for severe nausea or vomiting, since it is not absorbed via the stomach. The smelting tablet offers flexibility on the go.
The known brand name is Ascotop®.
Active Ingredients & Mechanism of Action
Active ingredient: Zolmitriptan
Zolmitriptan acts as a selective serotonin receptor agonist:
Active mechanism:
- Ties to 5-HT1B/1D receptors
- Verengt extended cranial blood vessels
- Inhibits the release of CGRP and other neuropeptides
- Blocks the trigeminal pain redirection

Initiation of action:
- Tablets: 45-60 minutes
- melting tablet: 45 minutes
- Nose spray: 15-30 minutes
Who is it suitable for?
Zolmitriptan is suitable for:
- Adults with acute migraine attacks
- Patients with pronounced nausea spray
- If other triptans do not work enough

Available Dosages
Tablets:
- 2,5 mg

melting tablets:
- 2,5 mg

Nasenspray:
- 5 mg per spray
Recommended dosage:
- Standard dose: 2.5 mg
- In case of insufficient action: 5 mg
- Repeat after 2 hours
- Maximum: 10 mg in 24 hours
How to Take
Tablets:
- Suck with water
- Take the start of headaches
melting tablets:
- Put on the tongue, let go
- Swallow with saliva
- No water needed
Nasenspray:
- Head slightly forward
- Spray into a nostril
- Do not breathe during spraying
- Keep head down a few seconds
General Notes:
- Taking first headaches
- Not during the aura
- Second dose only after 2 hours
Contraindications
**Zolmitriptan must not be taken at:* *
- coronary heart disease
- Heart attack, stroke, TIA in history
- Wolff-Parkinson-White Syndrome or other stimulus disorders
- Uncontrolled high blood pressure
- Heavy liver failure
- Hemiplegic or Basilaris marshes
- MAO-A inhibitor therapy
- Simultaneous ergotamine intake
Preview at:
- Medium liver function disorder (lower dose)
- Cardiovascular risk factors
- SSRI/SNRI therapy
Possible Side Effects
Acid (1-10%):
- Taste disorders (especially nasal spray)
- Paresthesia (cribing)
- Squeeze
- Bemonia
- Headaches
- Mouth dryness
- nausea
Triptane-specific sensations:
- Warmth
- pressure/envidentness in chest and neck
- Heavy in limbs
- Mostly harmless!
Nasenspray added:
- unpleasant taste
- Nasal irritation
- Nose bleeding
Selten:
- Allergic reactions
- Coronary seeds
- serotonin syndrome
Interactions
Not combine with:
- Ergotamine (24 hours distance!)
- Other triptans on the same day
- MAO-A inhibitors (Moclobemid)
Dosis reduction at:
- Cimetidine intake (max. 5 mg/24h)
- Fluvoxamin (max. 5 mg/24h)
Preview at:
- SSRI/SNRI (Serotoninsyndrom risk)
- St. John's wort
- Tryptophan
