What is Trimbow?
Trimbow is the first triple inhaler that combines three active ingredients in one device: the inhalative corticoid beclometason, the long-acting beta-2-agonist formoterol and the long-acting anticholinergic glycopyrronium (LAMA).
This triple therapy (ICS/LABA/LAMA) is used in patients who are not sufficiently controlled with a dual therapy (ICS/LABA). Trimbow is approved for the treatment of moderate to severe asthma and COPD.
The extra-fine formulation ensures optimal distribution even in the small airways. Trimbow is available as a dosing aerosol and as a Nexthaler (powder inhaler).
Active Ingredients & Mechanism of Action
Active ingredient: Beclometasondipropionat, Formoterol, Glycopyrroniumbromid
Trimbow combines three working mechanisms:
Beclometason (ICS):
- Anti-inflammatory action
- Reduces swelling and fog production
- Lower bronchial hyperreagiability
Formoterol (LABA):
- Fast bronchodilation (1-3 minutes)
- duration of action 12 hours
- Relaxes the bronchial muscle
Glycopyrronium (LAMA):
- Blocks Muslim receptors
- Additional bronchodilate
- duration of action up to 24 hours
- Reduced mucus secretion
Driving effect: The three mechanisms complement each other: Inflammation inhibition + double bronchodilatation via different paths.
Who is it suitable for?
Trimbow is suitable for:
- Adults with heavy asthma (if ICS/LABA is not enough)
- COPD patients with frequent exacerbations
- As a step-up of dual therapy

Not suitable:
- In light to moderate asthma (overtherapy)
- First-line therapy
- For children and young people under the age of 18
Available Dosages
Trimbow dosing aerosol (extrafein):
- 87/5/9 μg (Beclometason/Formoterol/Glycopyrronium) per stroke
Trimbow Nexthaler:
- 88/5/9 μg per single dose
Dosing:
- 2 strokes 2x daily
- Maximum: 4 strokes per day
** The extrafine formulation allows lower beclometason doses with the same activity.
How to Take
Dosieraerosol: 1. Remove protective cap, shake Two. Deep exhalation 3. Close the mouthpiece 4. Breathe slowly and deeply, spray Five. Keep breath 10 seconds 6. Second stroke just as 7. Rinse and sprinkle
Nexthaler: 1. Open protective cap Two. Breathe deep (not in the inhaler) 3. Close the mouthpiece 4. Breathe quickly and deeply Five. Keep breath 6. Rinse mouth
Important:
- 2x daily, morning and evening
- Apply regularly

Contraindications
Do not apply at:
- hypersensitivity to the active ingredients
Preview at:
- Engangle glaucoma
- Prostate hyperplasia
- Heavy cardiovascular diseases
- QT extension
- thyroid hyperfunction
- Diabetes mellitus
- Hypokaliemia
pregnant: Only with clear indication.

Possible Side Effects
Acid (1-10%):
- Mundsoor
- Heiserkeit
- Headaches
- urinary tract infections
- Mouth dryness
Occasionally:
- Tremor
- Heart palp
- Muscle cramps
- Hidden vision
- Harnverhalt
Selten:
- Systemic corticoid effects
- Engangle glaucoma
- Paradox bronchospasmus
Prevention: rinse the mouth after inhalation.
Interactions
Reactions:
- CYP3A4 inhibitors – low risk (Beclometason cheaper than Fluticason)
- Betablockers - can remove formoterol
- Other anticholinergics – additive effect
- QT-extending medication
- Potassium sinking drugs
Not recommended:
- At the same time other LABA or LAMA
