What is Rosacea?
Rosacea (also Rosazea) is a chronic and endogenous skin disease that affects the face. In Germany, for example, 2 to 5 percent of adults are affected, with light-hearted people suffering more frequently. The disease typically begins between the ages of 30 and 50.
Characteristic are facial ovations (Flushing), permanently extended apeeches (Teleangiectasien), infinite papules and pussulas as well as a burning or stinginging skin feeling. In contrast to acne, Rosacea has no mitesser (comedons).
The exact cause is not fully complained. A misregulation of the innate immune system, a strobed skin barrier, the demodex mite (a natural skin dweller that is increasingly present in Rosacea) and genetic factors play a role. Certain triggers can get rid of Schuebe.
The four subtypes of the Rosacea
Subtype 1 - Erythematoes-teleangiektatische Rosacea: Permanent Roetung (Erythem) in the central area of the face (wags, nose), visible veins and random flushing. The skin is often sensitive and reacts to many care products.
Subtype 2 - Papulopustuloes Rosacea: In addition to the Roetung, infinite papules and dolls occur, which remind of acne, but without Mitesser. This is the most widely treated subtype.
Subtype 3 - Phymatoese Rosacea: Thickening of the skin, especially on the nose (Rhinophym, umgangssprachlich Knollennase). Subject: Maenner. Operational or laser therapy may be required in advanced faellens.
Subtype 4 - Okulaere Rosacea: Participation of the eyes with dryness, burning, foreign coercion and conjunctural decommissioning. Can occur isolated or accompanying to skin symptoms and requires an immediate support.
Many patients show features of several subtypes simultaneously. The therapy is adjusted according to the prevailing symptoms.
Avoid triggers and optimize skin care
Identification and avoidance of perseveral triggers is an important therapy component:
Haeufige Trigger: UV radiation (most important trigger), sharp food, alcohol (especially red wine), hot drinks, temperature fluctuations (sauna, cold wind), stress and certain skin care products.
Skin care at Rosacea: Use mild, perfume-free cleaning products with neutral pH. Dispense with alcohol, menthol, fighters and coarse peelings in care. A light, oil-free moisturizer calms the skin. Temporary sun protection with at least LSF 30 (mineral filter preferred) is indispensable.
Green colour pigments: Special day creams with green pigments can optically neutralize roetungen. This cosmetic measure does not replace medical therapy, but can improve well-being.
Medicoesis treatment
Topic therapy (otherwise):
- Metronidazole cream/gel (0.75 %): Classic first-line medication for papulopustuloeser Rosacea. Works anti-inflation and antibacterial. Application one or two times over several months.
- Azelainsaure (15 %): Just as effective as Metronidazole with additional brightening effect in roetungen. Particularly suitable with simultaneous papules and erythema.
- Ivermectin cream (1 %): Works against the Demodex mite and anti-return. In studies, Ivermectin shows a slightly better activity than Metronidazole in papulopustuloeser Rosacea.
- Brimonidine gel (0.33 %): Reduces blood flow and reduces facial cleansing for 8 to 12 hours. Works purely symptomatic, not against papules or pussies.
Systemic therapy (internal): In the case of heavy rosacea, low dosed doxycycline (40 mg) can be prescribed correctly. In this low, sub-antibiotic dosage, it has a dispensing effect without foerdering antibiotic resistances. The duration of therapy is typically 8 to 12 weeks.
