What is olmesartan?
Olmesartan is a potent AT1 receptor blocker (Sartan) with a strong blood pressure-lowering effect. It is available as a mono preparation and in various combinations: Olmetec®, Olmetec Plus® (with HCT), Sevikar (with Amlodipine) and Vocado® HCT.
Olmesartan is often used when a strong reduction in blood pressure is required and can be combined well with other active ingredients.
Active Ingredients & Mechanism of Action
Active ingredient: Olmesartan medoxomil
Olmesartan blocks the AT1 receptor:
Active mechanism:
- Selective and potent AT1 receptor blockade
- Used as Prodrug (Olmesartan medoxomil)
- Intestine converted to active Olmesartan
- Strong and persistent blood pressure reduction
Clinical effect:
- belongs to the more blood pressure-lowering sartans
- duration of action: 24 hours
- Good protection even in the morning hours
Who is it suitable for?
Olmesartan is suitable for:
- Adults with high blood pressure
- Patients who need more blood pressure reduction
- In combination with amlodipine and/or HCT
Especially suitable for:
- Treatment-resistant high blood pressure
- Need for combination therapy
Not suitable:
- Pregnancy and breastfeeding
- Gallenwegsobstruction
- Heavy liver failure
Available Dosages
High blood pressure:
- Start dose: 10 mg once a day
- usual dose: 20 mg once a day
- Maximum dose: 40 mg daily
Available starches:
- 10 mg, 20 mg, 40 mg tablets
Combination preparations:
- Olmesartan + HCT (Olmetec Plus®)
- Olmesartan + Amlodipine (Sevikar)
- Olmesartan + Amlodipin + HCT (Vocado® HCT)
How to Take
Intake:
- Once a day, always at the same time
- Independent meals
- Take with water

- Regular blood pressure control
- Laboratory controls (animal, potassium)
- Don't suddenly stop
- Drink enough
Contraindications
**Olmesartan must not be taken at:* *
- hypersensitivity to Olmesartan
- Pregnancy and breastfeeding
- Gallenwegsobstruction
- At the same time with aliens in diabetes
Preview at:
- Kidney failure
- Kidney artery stenosis
- Hyperkaliemia
- Dehydration
- Aortenstenose
Special warning: In rare cases, Olmesartan can cause a spue-like enteropathy (heavy diarrhea with weight loss). In case of chronic diarrhea in Olmesartan inform the doctor.
pregnant: contraindicated throughout pregnancy.
Possible Side Effects
Acid (1-10%):
- Squeeze
- Headaches
- Increased blood fat levels
** Occasionally (0,1-1%):* *
- fatigue
- Gastrointestinal complaints
- Hyperkaliemia
- Kidney malfunction
- Husten (rare)
Selten, but important:
- Sprue-like enteropathy (chronic diarrhea, weight loss)
- Angioedema
- Liver malfunction
** In the case of chronic, unexplained diarrhea in Olmesartan, a spue-like enteropathy should be considered. After decommissioning, the symptoms are better.
Interactions
Contracted:
- Aliskiren in diabetes or kidney failure
Preview at:
- Potassium-saving diuretics → Hyperkaliemia
- NSAR → reduced effect
- Lithium → raised mirrors
- Colesevelam → reduced Olmesartan recording
** Colesevelam (Gallen Acid Binder) can reduce the absorption of Olmesartan. Time distance recommended.
