What is Mysimba?
Mysimba is an oral drug for weight reduction that combines two active ingredients: Naltrexon (an opioid antagonist) and Bupropion (an antidepressant/smoothing agent).
Unlike the GLP-1 agonists (Wegovy, Ozempic), mysimba acts primarily in the brain on the reward and saturation center. It is taken as a tablet, not sprayed.
Mysimba is approved for adults with BMI ≥ 30 or BMI ≥ 27 with accompanying diseases.
Active Ingredients & Mechanism of Action
Active ingredient: Naltrexon 8 mg, Bupropion 90 mg
The combination has a synergistic effect on the brain:
Bupropion:
- Inhibition of dopamine and norepinephrine
- Stimulates neurons in the hypothalamus that regulate appetite
- Reduces the desire for food
Naltrexon:
- Blocks Opioid receptors
- Strengthens and extends the action of Bupropion
- Reduces the feeling of reward when eating
Call:
- Reduced appetite
- Reduced desire for food (especially "emotional eating")
- Increased energy consumption
Who is it suitable for?
Mysimba is suitable for:
- Adults with BMI ≥ 30 (obesity)
- Adults with BMI ≥ 27 and at least one weight-related accompanying disease
- People who have difficulties with emotional food
- As an alternative to injectable drugs
Not suitable for uncontrolled hypertension, epilepsy, eating disorders.
Available Dosages
Stepwise dose increase:
- Week 1: 1 tablet in the morning
- Week 2: 1 tablet in the morning + 1 evening
- Week 3: 2 tablets in the morning + 1 evening
- From week 4: 2 tablets in the morning + 2 in the evening (maintenance dose)
Maximaldosis: 4 tablets daily (2-0-2)
How to Take
- Swallow pills with water
- Preferably at meals
- Not with high-fat meal (increase risk)
- Dosage increase as prescribed
Contraindications
Absolute contraindications:
- Uncontrolled high blood pressure
- Epilepsy or seizures in prehistory
- tumor of the central nervous system
- alcohol or benzodiazepine withdrawal
- Bulimia or Anorexia
- Simultaneous intake of MAO inhibitors
- Opioid intake or withdrawal
- Heavy liver failure
Relative contraindications:
- Bipolar disturbance
- Heavy depression
- Kidney failure
Possible Side Effects
Very common:
- nausea
- Blocking
- Headaches
- Break.

- Squeeze
- Mouth dryness
- Sleep disorders
- Fear, irritability
- Blood pressure increase
- stomach pain
Selten, but seriously:
- seizures (risk increases with rapid dose increase)
- Suicidal thoughts (monitoring in psychiatric history)
Interactions
Contracted:
- MAO inhibitor (14 days distance)
- Opioide (Naltrexon blocks its effect!)
- Other Bupropionic preparations
Preview:
- Blood pressure-enhancing drugs
- medicines that lower the cramp threshold
- CYP2D6 substrates (tamoxifen, metoprolol)
