What is Famciclovir?
Famciclovir is an antiviral drug for the treatment of herpes infections. It is a prodrug that is converted into penciclovir in the body and acts against herpes-simplex viruses (HSV-1 and HSV-2) as well as the Varizella-Zoster virus.
In Genitalherpes, Famciclovir offers similar advantages as Valaciclovir: good uptake, simple dosage scheme and high efficiency. It is an alternative for patients not tolerate Valaciclovir.
Known brand name is Famvir®.
Active Ingredients & Mechanism of Action
Active ingredient: Famciclovir
Famciclovir is activated in the body to Penciclovir:
Conversion: 1. Famciclovir is converted into penciclovir in the intestine and liver Two. Penciclovir is phosphorylated in infected cells by viral thymidine kinase 3. Penciclovir triphosphate inhibits viral DNA polymerase 4. The virus DNA synthesis is blocked

Who is it suitable for?
Famciclovir is suitable for:
- Genital herpes – initial infection
- Genital herpes – recurring episodes
- Suppression therapy for frequent recurrents
- Belt rose (Herpes zoster)
Advantages:
- Alternative to Valaciclovir
- Well tolerated
- Easy dosing scheme
Restrictions:
- Doesn't heal herpes (virus stays in the body)
- Does not work against other viruses
- Dose adjustment required for kidney failure
Available Dosages
Genital herpes – First infection:
- 250 mg 3x daily for 5-10 days
Genital herpes – Rezidiv:
- 125 mg 2x daily for 5 days OR
- 1000 mg 2x daily for 1 day (short time therapy)
Suppression therapy:
- 250 mg 2x daily
Gurtelrose:
- 500 mg 3x daily for 7 days
For renal insufficiency: Dose reduction required depending on creatinine clearance.
How to Take
Intake:
- With or without food
- With sufficient water
- Regular distances between cans
For herpes outbreak:
- Start as early as possible (at first symptoms)
- Ideally within 24 hours after the start of symptoms
Suppression therapy:
- Daily at the same time
- Don't forget – Gaps reduce effectiveness
- Exhaust test after 6-12 months possible

- Do not forget sun protection (low photosensitization)
Contraindications
**Famciclovir must not be used at:* *
- hypersensitivity to Famciclovir or Penciclovir
Dosis adjustment at:
- Kidney insufficiency (important!)
- Liver insufficiency
- Older patients
pregnant: Only if necessary. Less data than available for Aciclovir/Valaciclovir.
Style time: Not recommended (unknown whether it passes into breast milk).
Possible Side Effects
Acid (1-10%):
- Headaches
- nausea
- diarrhea
Occasionally:
- Squeeze
- fatigue
- stomach pain
- Skin rash
Selten:
- confusion (in older patients, kidney failure)
- Hallucinations
- Heavy skin reactions
* *Compared to Aciclovir:** Similar side effect profile. Gastrointestinal complaints can be a bit more common.
Interactions
Reactions:
- Probeecid – increases Penciclovir mirror
- Other nephrotoxic medicines – additive kidney stress
No relevant interactions with:
- Hormonal contraception
- Most popular medicines
For renal insufficiency: Especially important: dose adjustment required.
